Internship Forms
Evans School internship forms, except for the internship waiver, are online forms. For the internship waiver you will need Adobe Acrobat Reader to view it.
- Agency/Student Contract Form
- Describes the scope of work, your professional objectives, and the agency’s expectations
- Must be submitted within two weeks of starting your internship
- Internship Evaluation Form
- Describes the quality of the internship and what you got out of it
- Must be submitted within two weeks of completing the internship
- Intern Supervisor Evaluation Form
- Remind your supervisor to complete and submit the form
- Must be submitted to Career Services during your last week of work
